Schedule your GUIDE Welcome Call

During this first call, you’ll speak with one of our Care Navigators, who specializes in supporting families living with dementia.

Together, you will:

  • Talk about your loved one’s history, daily routine, and current challenges
  • Share what’s been hardest for you as a caregiver
  • Start outlining a personalized care plan through the GUIDE program
  • Ask any questions you have about services, support, or what comes next

This is a conversation, not a test. Our goal is to understand your situation and begin making things easier, step by step.

Call length: Usually 30–45 minutes

Who should join: You, and any other family members or caregivers you’d like to include
