Lizzy Care brings this support to your home, tailored to your needs, at no cost for those with Traditional Medicare.

There is no cost for families with Traditional Medicare Parts A & B who live at home.

Am I Eligible?

You may be eligible if…

  • Traditional Medicare, Parts A & B, is your primary insurance.
  • You have a diagnosis of dementia or probable dementia. (If you don’t have a diagnosis yet, we can help you get one.)
  • You live at home, or in an independent living setting.

You may not be eligible if…

  • Your primary insurance is Medicare Advantage or another Medicare health plan.
  • You are residing in a long-term care, memory care, skilled nursing, or residential care facility.
  • You are participating in hospice or a PACE program.

GUIDE is an ongoing program. As long as you stay eligible and have Traditional Medicare, your services will continue. You don’t need to re-apply each year. Your care team will check in regularly and update your care plan as needed.

A caregiver is a family member, friend, or other unpaid person who regularly helps someone with dementia. This could be a spouse, adult child, or anyone close to them. You don’t have to live together to be a caregiver. GUIDE support, including education and respite, is made for caregivers.

Respite gives caregivers a break while a trained aide cares for your loved one. GUIDE offers respite support each year, so you can rest, recharge, or take care of other things, knowing your loved one is safe.

For families with Traditional Medicare (Parts A & B) as their main insurance, GUIDE services are fully covered by Medicare. There is no cost to you.

We work with your current doctors, not instead of them. Your primary care provider is still at the center of your loved one’s care. Our team helps with dementia care, coordinates with your doctor, supports your care plan, and acts as an extension of their office. If you don’t have a primary care provider, we can help you find one.

If you’re not eligible for GUIDE, Lizzy Care can still help your family with private-pay options. We’ll always explain what Medicare covers and what would be extra before starting any services. See our full list of services.
